To ensure compliance with NCQA, Quartz is required to survey our contracted providers annually to ensure our provider directory is accurate and our network providers’ access and availability falls within NCQA standards and Quartz policies.
We have incorporated all regulatory questions/surveys/verifications we are required to obtain from contracted providers on an annual basis into a single application, the Facility Operations Form. Someone at your facility should have recently received an email request to log into MyPlanTools to update this information. We appreciate your partnership in completing this questionnaire to make sure we are compliant with NCQA and other state and federal requirements.
Note that while we transitioned your provider portal to My Quartz Tools last year, the FOF is completed in MyPlanTools.
