What is Dual Eligible (D-SNP)?
D-SNP stands for Dual Special Needs Plan. These plans are for people who qualify for both Medicare and Medicaid. Quartz’s D-SNP plan is called Quartz Dual Eligible with Rx.
If you have Medicare and Medicaid, a Quartz Dual Eligible with Rx plan may help you get more benefits and support in one plan. With Quartz Dual Eligible, you get all of the benefits of Original Medicare, plus extra benefits and support. You may pay $0 for Medicare-covered services. How much Medicaid covers depends on your income, resources, and other factors.
To join Quartz Medicare Advantage, you need to be entitled to Medicare Part A, enrolled in Medicare Part B, and be a U.S. citizen or lawfully present in the U.S. To qualify and remain enrolled in a Quartz Dual-Eligible plan, you need to be eligible for Medicare and receive full Medicaid benefits or be considered a Qualified Medicare Beneficiary, and live in one of the following Wisconsin counties: Dane and La Crosse.
*You may be responsible for coinsurance if you lose your full Medicaid benefit and are no longer a qualified Medicare beneficiary. For some Dual Eligible Medicaid members, small copays may occur under their Medicaid benefit.

Am I eligible to enroll?
To qualify and remain enrolled in a Quartz Dual Eligible plan, you need to be eligible for Medicare, receive full Medicaid benefits, be considered a Qualified Medicare Beneficiary, and live in one of the following Wisconsin counties:
- UW Health Quartz Dual Eligible with Rx – Dane County
- Gundersen Quartz Dual Eligible with Rx – La Crosse County
When can I enroll in a Quartz Dual Eligible plan?
- When you’re eligible for Medicare (age 65): You can enroll during Medicare’s Annual Enrollment Period (AEP), which happens every year from Oct. 15 – Dec. 7
- If you become entitled to a Dual Eligible plan: You can enroll in a Quartz Dual Eligible plan any time you become eligible for the plan (based on your life circumstances), or at the beginning of any calendar quarter
- If you already have Medicare and Medicaid, you can enroll or switch Dual Eligible plans once per quarter (every three months) or during Medicare’s Annual Enrollment Period

What is a Dual Eligible Special Needs (D-SNP) plan ?
Dual Eligible plans, also known as Dual Eligible Special Needs (D-SNP) plans, are a kind of Medicare Advantage plan for people who qualify for both Medicare and Medicaid. These plans cover hospital, medical, and prescription costs – all in one plan, with little or no extra monthly costs.
What does “with Rx” mean?
“With Rx” means that the plan includes prescription benefits and coverage.
How do I keep my Dual Eligible plan from year-to-year?
As long as you continue to maintain proper Medicaid eligibility, you’ll stay qualified for a Dual Eligible plan. And, once enrolled, your coverage will automatically renew each year unless there is a change to your plan’s service area.
