Common new member questions
Quartz members receive their member ID card in the mail. If you have not received yours yet, we recommend you get in touch with our Customer Success team at (800) 362-3310.
Note: If you are a BadgerCare Plus or Medicaid SSI member, your ForwardHealth ID card is the card you will use to get your benefits.
Log in to your Quartz MyChart to find information about your benefits, medical and pharmacy coverage, claims, and billing information. You will need your Quartz member ID number to log in. If you do not know your ID number, our Customer Success team can help you find it (you can find the phone number of your dedicated team on the back of your ID card). You can also visit QuartzMyChart.com or download the Quartz MyChart app for access on the go.
Note: If you’re a Quartz Align member, you will utilize the MyCreateHealth portal to locate this information.
You can use our Find a Doctor tool to find a health care provider or medical facility. This tool helps you locate the care you need in our network of top-ranked providers. Search by location, provider name, specialty, or medical service needed. Whether you need to schedule an MRI or simply locate the nearest urgent care facility, Find a Doctor is the way to go.

Managed care is designed to help keep members healthy. Care Management Programs coordinate the care and services for members who need extra support with their medical and social needs. Managed care empowers members to seek preventive health care services and to work with their primary care provider (PCP) to make the appropriate health care choices. You can contact our Care Management team at (866) 884-4601.
If your plan includes pharmacy benefits, you can read about them by logging into Quartz MyChart and accessing the pharmacy benefits tool.
Note: BadgerCare Plus & Medicaid SSI members receive their pharmacy benefits through the state. You may get a prescription from a Quartz network doctor, specialist, or dentist. You can get covered prescriptions and certain over-the-counter items at any pharmacy that will accept your ForwardHealth ID card.
Quartz Align members also receive access to the MyCreateHealth portal to locate this information.

You can learn about your member rights and responsibilities in our Quartz’s Member Rights and Responsibilities Statement. It explains our commitment to a mutually respectful relationship with our members and practitioners.
As a Quartz member, you can allow someone else to access your Protected Health Information (PHI). This authorization could be helpful if you want someone else to be able to communicate with Quartz on your behalf, such as inquiring about a claim or prior authorization. To do so, complete the Authorization to Release Protected Health Information (PHI) form and submit it to Quartz.
You can fill out and submit online the Authorization for Disclosure of Protected Health Information form at QuartzBenefits.com/PHIform or in Quartz MyChart. This lets you choose which information can be shared with specific individuals or organizations. Once submitted, this form is valid for two years.
