Self-Management Equipment

At Quartz, we strive to assist our members in actively managing their chronic conditions. To support this, we will provide members with a qualifying condition, an automatic blood pressure monitor, or a scale at no extra cost to complement their health care team’s education. Members with hypertension qualify for a blood pressure monitor, whereas members with obesity, diabetes, pre-diabetes, hypertension, or congestive heart failure qualify for a home scale.

To order self-monitoring equipment for your patient, fill out the self-monitoring equipment order form. Fax the completed order form with documentation to UW Health Care Direct.
