The 1095-B Form will detail your 2016 coverage and should be kept with your tax records.
In January, we will send you a printed copy of your 1095-B Form* and it will also be available in MyChart.
If you don’t want to receive a printed copy in the mail, please update your preferences in MyChart by January 10, 2017. We will email you when the form is available to print from MyChart.
* Note: BadgerCare Plus, Health Insurance Marketplace (HIM) and Medicare members
may receive a similar form from Medicaid, HIM or Medicare. They will not receive a 1095-B form from Unity.
