Quartz Well provides many ways to earn points you can redeem for rewards.3 If you used a fitness tracker in 2018 to log your activities, you were credited with 2,000 points. To help keep the healthy rolling into
the new year, we’ll add another 2,000 points to your Quartz Well balance in January 2019. That’s enough points for a $50 Amazon gift card.

  • If you’re already synced, you’re all set. Just keep your device connected and keep moving. We’ll do the rest.
  • If you’re not synced yet, there’s still time in 2018 to connect, start tracking, and earn points.

Remember — Quartz Well and device syncing are accessible only through your MyChart account.

Redeem Your Points

Don’t forget about your Amazon code! If you earned enough points for an Amazon gift card, make sure you go get your Amazon code before it goes away December 31, 2018.

Here’s what you need to do —

  • Log in to MyChart and go to your Quartz Well portal. In the middle of the
    screen you’ll see your Amazon code and a little green envelope.
    Don’t see your code or envelope icon? Just click on the “Claim Your Gift Card” link first.
  • Click on the envelope to email the code to your personal email account.
  • Then, grab that code from your email, go to Amazon and redeem your points. It’s that easy!

Remember, once you’ve emailed your code to yourself, it won’t expire. So, save your Amazon code today. You’ve earned it!

Preventive Screening / Quartz Well points

Did you have a preventive screening in late 2018? You earned Quartz Well points if you did.

If these points don’t show up in 2018, they’ll be credited to your 2019 Quartz Well rewards. It all depends on when your
provider bills us.

  1. The Quartz Well reward program is not available to State of Wisconsin Group Health Insurance Program members.
  2. The Quartz Well reward program is not available to members of health plans underwritten by Physicians Plus Insurance Corporation.
  3. Subscribers age 18 and older can earn $100 for single plans. Family plans offer $100 for the subscriber and $100 for the subscriber’s spouse (or domestic partner) depending on your plan.
Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to members age 18 and older. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact us at (800) 362-3310 and we will work with you (and if you wish, your doctor) to find a wellness program with the same reward that is right for you in light of your health status.
