With Quartz Dual Eligible, you get all the benefits of Original Medicare, plus more. If you have full Medicaid benefits and are a qualified Medicare beneficiary, you will pay $0 for your Medicare-covered services. 

You’ll also have access to all the doctors, specialists, and hospitals of nationally recognized providers, such as Aurora Health Care, Gundersen Health System, and UW Health. Plus, you get access to even more doctors across Illinois, Iowa, Minnesota, and Wisconsin.

Am I eligible to enroll?

To join Quartz Medicare Advantage, you need to be entitled to Medicare Part A, enrolled in Medicare Part B, and be a U.S. citizen or lawfully present in the U.S. To qualify and remain enrolled in a Quartz Dual Eligible plan, you need to be eligible for Medicare and receive full Medicaid benefits and be considered a Qualified Medicare Beneficiary, and live in one of the following Wisconsin counties:

  • UW Health Quartz Dual Eligible – Dane County
  • Gundersen Quartz Dual Eligible – La Crosse County
  • Aurora Health Quartz Dual Eligible – Milwaukee County
  • If you’re eligible for Medicare, you can enroll during the Medicare Annual Enrollment Period, which happens every year from Oct. 15- Dec. 7. 
  • If you’re entitled to a Dual Eligible plan, you can enroll in a Quartz Dual Eligible plan now or at the beginning of any calendar year quarter. 
  • If you already have Medicare and Medicaid, you can enroll or switch Dual Eligible plans once per quarter (every three months) or during the Medicare Annual Enrollment Period. As long as you maintain proper Medicaid eligibility, you’ll stay qualified for a Dual Eligible plan. Once enrolled, your coverage will automatically renew each year unless there is a change to the plan’s service area.

You can apply for or enroll in a Quartz Dual Eligible plan one of two ways:

Dual Eligible plans, also known as Dual Eligible Special Needs (D-SNP) plans, are a kind of Medicare Advantage plan for people who have both Medicare and Medicaid. These plans cover hospital, medical, and prescription costs – all in one plan, with little or no extra monthly costs.

“With Rx” means that the plan includes prescription benefits and coverage.

To join a Dual Eligible plan, you need to be entitled to Medicare Part A, enrolled in Medicare Part B, and be a U.S. Citizen or lawfully present in the United States.

You must live in one of the following Wisconsin counties to enroll in a Quartz Dual Eligible plan: 
  • Dane County for UW Health Quartz Dual Eligible
  • La Crosse County for Gundersen Quartz Dual Eligible
  • Milwaukee County for Aurora Health Quartz Dual Eligible
You have three opportunities to enroll in a Quartz Dual Eligible plan:
  1. When you’re eligible for Medicare (age 65): You can enroll during Medicare’s Annual Enrollment Period (AEP), which happens every year from Oct. 15 – Dec. 7 
  2. If you become entitled to a Dual Eligible plan: You can enroll in a Quartz Dual Eligible plan any time you become eligible for the plan (based on your life circumstances), or at the beginning of any calendar quarter
  3. If you already have Medicare and Medicaid, you can enroll or switch Dual Eligible plans once per quarter (every three months) or during Medicare AEP

As long as you continue to maintain proper Medicaid eligibility, you’ll stay qualified for a Dual Eligible plan. And, once enrolled, your coverage will automatically renew each year unless there is a change to your plan’s service area.
