Quartz MyChart Instant Activation Now Available

MyChart is a secure and convenient way to help manage the health care needs for you and your family. Now, members can activate MyChart accounts instantly at unitymychart.com.

Look for the Request an Account button and give it a click. After answering security questions to verify your identity, you can instantly create your individual login and password. If you’d rather sign up through your clinic, activation codes are also available from clinic reception staff or you can request one online.

As a MyChart member you can –

  • View your benefit information
  • Check claims status
  • Check eligibility
  • Review prior authorizations
  • Sign up for email messages from Unity including your explanation of benefits
  • Take a test to see how healthy you are and access your very own wellness portal
  • Request ID cards
  • Change your Primary Care Physician
  • Change your address, phone number and more
  • Send a message to Unity Customer Service

If you visit a UW Health provider, you can –

  • Get your test results online
  • Make, cancel and view appointments
  • View after visit summaries
  • Read and print articles about health issues, medicines, allergies and immunizations
  • Send secure messages to your health care team
  • See information above for your children (All features are available for children ages 0-11. For children ages 12-17, access is restricted to allergies, immunizations, sending messages and scheduling)
