Quartz offers many kinds of prescription drug coverage.

To review your formularies, choose from the options below based on your plan. If you are unsure of what plan you have, please contact Quartz Customer Success at (800) 362-3310.

You can also view your drug formulary here. The formulary lists the drugs covered on the drug benefits.

Standard formulary

Select this formulary if you have a high-deductible (HSA), tier 1 or tier 2, or coinsurance benefit plan.
Standard Formulary

Standard Choice formulary

Select this formulary if you have a tier 3 or tier 4 plan or if you have a Quartz Align (ASO) plan.

Standard Choice Quality formulary

Select this formulary if you have a plan that includes the value tier.

Quartz does not provide pharmacy benefits for:

  • State and local government members 
    Please visit Navitus for information about your prescription drug benefits.
  • BadgerCare Plus members
    Please call the Department of Health and Family Services at (800) 362-3002 for information about your prescription drug benefits.
  • Medicare Advantage members who did not select a plan with Part D (prescription drug) coverage
  • Medicare Select members
  • Medicare Supplement members
