Make the most of your benefits

Start using your coverage today. Here are some tips to help:

• Use your ForwardHealth ID card to get your Quartz BadgerCare Plus and Medicaid SSI benefits. Take it to doctor appointments and when you’re getting a prescription filled. Medicaid SSI members will also get a Quartz member ID card. Keep both cards with you as you need them.
• Choose a primary care provider (PCP) to take care of your health and refer you to a specialist, if needed. To find a PCP in network and local to you, members can use the following tool:

  Find a Doctor – Medicaid SSI members
  If you need help choosing or changing your PCP, call our Customer Success team at (800) 362-3310 (TTY: 711).
• Review the Quartz BadgerCare Plus and Medicaid SSI Member Handbook for details about your coverage. This includes information about extra benefits and services that are part of your plan. Call Quartz Customer Success at (800) 362-3310 (TTY: 711) if you would like a printed copy of the handbook.
• Review your member rights and learn how Quartz protects your private information.

Completing your Health Risk Assessment (HRA)

We ask each member of your household who qualifies for BadgerCare Plus or Medicaid SSI to complete an HRA within 90 days of enrollment. You should also take an HRA once each year to help us provide coverage that fits your needs.
