This measure is vital for enhancing cardiovascular health outcomes and carries a threefold weight in Medicare Advantage Star Ratings.

Providers should focus on ensuring accurate documentation of blood pressure readings in patient records, scheduling follow-up appointments for patients with elevated readings, conducting in-office rechecks for patients who previously recorded high blood pressure to provide accurate measurements, and educating patients about the significance of managing their blood pressure through lifestyle modifications, medication adherence, and regular check-ups.

Encouraging the use of self-monitoring blood pressure cuffs at home can also enhance patient engagement and awareness, empowering individuals to take an active role in their health management. Home blood pressure readings reported by the patient should be noted by the provider and accurately documented in the EMR to assist with blood pressure management.
