Dental Wellness
Get preventive and comprehensive dental coverage with a Delta Dental network provider who also accepts Medicaid. $3,000/year annual limit.

Obesity and Type 2 Diabetes Reversal Benefit
Eligible Quartz Dual Eligible members can join the Obesity and Type 2 Diabetes Reversal program from Virta Health. This program can help members lower blood sugar, reduce the need for certain medications and achieve a healthier weight.
Virta is available at no extra cost* to Quartz Medicare Advantage and Dual Eligible members with Type 2 diabetes. For members interested in safe, sustainable weight loss, Virta is also covered for those with a BMI of 30 or greater.
*Additional costs may be incurred for labs obtained from an out-of-network provider based on contractual agreements between the lab and Virta Health.

UWHealth Care Anywhere lets you quickly connect with an in-network provider 24/7 from the comfort of your home or wherever you are. No appointment is needed. Getting the urgent care you need is a click away with your smartphone, tablet, or computer equipped with a web camera – all with a $0 copay.

Quartz Nourishing Meal Program
The Quartz Nourishing Meal Program benefit helps to support your food and nutrition needs. It is available to Quartz Dual Eligible members upon discharge from an inpatient or skilled nursing facility stay. It is also provided to members with congestive heart failure enrolled in a Quartz Care Management program.
The Nourishing Meal Program is a partnership with Foodsmart and offers:
- Home-delivered meals made specifically for your individual health needs
- Telenutrition visits with a registered dietitian
- A personalized food and nutrition plan
Blood glucose meters and test strips
OneTouch® blood glucose meters and test strips
Get access to high-quality blood glucose meters with Bluetooth capabilities that connect to the OneTouch Reveal mobile app, which automatically logs readings, tracks patterns, and enables members to share data with their care team. Our preferred brand of blood glucose monitoring supplies is the OneTouch® product line, which includes the OneTouch Verio Reflect® with backlit display, Blood Sugar Mentor™, One Touch Verio Flex®, OneTouch Ultra Plus Flex®, OneTouch Ultra 2® meters, and their associated test strips. All are available at no extra cost.
Personal Emergency Response System
You have access to a 24/7 wearable monitoring Personal Emergency Response System. Maintain your independence while knowing help is near in the event of an emergency.
Eligibility for a Personal Emergency Response System is based on, but not limited to: living situation, living alone, history of falls, mobility and fall risk, and medical diagnoses.
Travel Benefit
This plan travels with you! For three months at a time, up to six months a year, get covered services at in-network costs when traveling nationwide outside of the state of Illinois, Iowa, Minnesota, and Wisconsin. Plus, members have access to most retail pharmacies nationwide.
