Effective Jan. 1, 2025, as required under HB 5395: Health Care Protection Act, Quartz requires participating providers within Illinois to notify us within 20 business days of deciding to cease accepting new patients if the new patient limitation is expected to last 40 business days or longer.
All other providers are required to notify us within 60 days prior to the provider not accepting new patients. As a reminder, we require providers to notify us within 10 business days of any changes to their information.
The Practitioner Notification Form can be utilized to inform Quartz of such changes.
