ESRD Documentation

Do you work with End Stage Renal Disease (ESRD) patients? If so, make sure you’re submitting the proper information to CMS per DHHS regulations.

The provider’s responsibility is to submit form CMS-2728, the ESRD Medical Evidence Report Medicare Entitlement and/or Patient Registration. This form provides CMS with necessary ESRD data, which in turn drives ESRD-based Medicare eligibility. It is critical that you do your part to ensure ESRD information is submitted to CMS.
