Medicare is federal government health insurance for people 65 and older or select individuals who have been on Social Security disability for at least two years.

There are different types of Medicare plans: 

  • Medicare Original (Partes A y B)
  • Planes Medicare Advantage (Parte C)
  • Planes de medicamentos recetados (Parte D)
  • Planes complementarios de Medicare
  • Dual Eligible Special Needs plans

Original Medicare is provided through the federal government and is made up of two parts:

Medicare Part A covers hospital stays. There’s no monthly premium, but you might have to pay a deductible.

Medicare Part B covers doctor visits and urgent care visits. There is a monthly premium, based on income, that is usually deducted directly from your Social Security check. You may still have to pay for some services, including a deductible.

Things to keep in mind:

    • Many people find that Original Medicare may not be enough.
    • Original Medicare does not include prescription drug coverage

Medicare Advantage plans (Part C) These plans are offered by private health insurance companies.

    • Includes all the benefits of Original Medicare, plus additional coverage, and extra benefits.
    • Some Medicare Advantage plans include prescription drug coverage.

Things to keep in mind:

    • You must be enrolled in Original Medicare before you can sign up for a Medicare Advantage plan.
    • In addition to your Part B premium, you may have to pay an additional monthly premium, although some plans have a $0 premium

Prescription Drug plans (Part D) These plans are offered by private health insurance companies and focus exclusively on prescription drug coverage.

    • Covers many commonly used brand name and generic drugs.
    • Copays can vary by plan and prescriptions.

Things to keep in mind:

    • You pay a monthly premium.
    • Available as a stand-alone plan but also may be included in Medicare Advantage plans.

Medicare Supplement plans  are also offered by private health insurance companies. They are also called Medigap plans because they help cover the costs, or “gaps,” not covered by Original Medicare. 

  • Covers the same benefits as Original Medicare (Parts A and B).
  • Original Medicare covers 80% of covered doctor and hospital visits, and Medicare Supplement plans cover the remaining 20%, plus the Part A deductible. (BOLD)

Things to keep in mind: 

    • You pay a monthly premium in addition to your monthly Part B premium. 
    • Plans do not include prescription coverage.
    • Typically, these plans do not offer the same level of additional benefits (such as hearing and vision) as Medicare Advantage plans.

To enroll in Original Medicare: If you’re going to collect Social Security retirement benefits when you turn 65, you’re likely to be automatically enrolled in Part A, and probably Part B.

  • Enroll in Part A as soon as you’re eligible 
  • If your employer (or your spouse’s employer) has more than 20 employees, and the group health plan is not through active employment, you may need to enroll in Medicare Part B to avoid penalties 
  • If your employer (or your spouse’s employer) has fewer than 20 employees, you may want to enroll in Medicare Parts A and B. Medicare will be the primary payer, and the group health plan will be secondary 
  • Ask your employer (or your spouse’s employer) if their plan’s prescription drug coverage is as good as a Medicare plan. If not, enroll in a Part D plan to avoid penalties 
  • Compare the total of your new monthly premiums and coverage to a Medicare plan to see which offers the better value 

If you DO NOT want to keep your employer coverage (or stay on your spouse’s employer coverage) 

  • Talk to your employer (or your spouse’s employer) about options or plans available to employees age 65 and older 
  • Enroll in Medicare Parts A and B when you are eligible 
  • Decide if you want to enroll in a Medicare Advantage plan, Medicare Supplement plan, or see if you qualify for a Dual Eligible plan 
  • If you have prescriptions, sign up for a Prescription Drug plan (Prescription drug coverage may be included with a Medicare Advantage or Dual Eligible plan)

If your current health insurance was purchased through the marketplace 

  • If you have health insurance from the health insurance marketplace and not from an employer, you’ll need to enroll in Medicare Part A and Part B when you’re eligible at age 65. 
  • You do not need to drop your marketplace insurance to enroll in Medicare, but most people do. 
  • For those who qualify for premium-free Medicare Part A, your marketplace insurance premium subsidies end after you’re eligible for Part A at age 65
