As a Quartz member, you have access to tailored support for your health and well-being. Your Quartz Well Health Coach makes it easier to find the care and resources you need at no extra cost to you. They can help with: 

• Create an individualized health plan, addressing goals, support systems, and barriers
• Assist with establishing or connecting with primary care provider, as needed
• Review their EPIC medical record for any needed preventive health screenings (i.e., annual exam, mammogram, colonoscopy)
• Help with Quartz Well Rewards navigation, including earning points, syncing a device, and redeeming rewards
• Arrange follow-up sessions, as desired.

Following the consultation, participants will be guided to complete their Quartz Well Health Assessment and provided an individual report including a personal health score, resources, and recommendations. Completing the Health Assessment is worth 3,000 wellness points.
