Quartz contracts with the Wisconsin Department of Health Services (DHS) to offer BadgerCare Plus and Medicaid SSI HMOs. The DHS takes care of some plan services for BadgerCare Plus and Medicaid SSI members. See the list below.

ForwardHealth ID Card
If you have questions about your ID card or you need a new ID card, call ForwardHealth Member Services at (800) 362-3002.
Enrollment
If you have enrollment questions, call a ForwardHealth HMO Enrollment Specialist at (800) 291-2002.
Pharmacy
Some drug stores accept ForwardHealth ID cards. If so, members can get covered prescriptions. They may also cover some other medicines.
The State of Wisconsin takes care of these services. BadgerCare Plus members Please call the Department of Health at (800) 362-3002 for information about your prescription drug benefits.
Dental, Chiropractic, and Autism Treatment Services

You need to find a doctor or clinic that accepts your ForwardHealth ID card. To find a doctor or clinic, call ForwardHealth Member Services at (800) 362-3002. The State of Wisconsin takes care of these services.
Dental Benefits for Members in Milwaukee County
For BadgerCare Plus and Medicaid SSI members living in Milwaukee County, Quartz provides all dental benefits through Momentum. In Milwaukee County, you must go to an in network dentist.
