Quartz no longer provides BadgerCare Plus ID cards. Although we’re still required to inform members of their PCC/PCP information, we now communicate this in the new member/continuing member letters.
- BadgerCare members now use their Forward Health ID card going forward.
- Use the Forward Health portal to verify which HMO the member has.
- All remaining Quartz ID cards will have Primary Care Clinic (PCC) removed.
