Open Enrollment

The Open Enrollment is the time period each year when individuals can review their plan options and make any needed changes for the following year. Outside of Open Enrollment, individuals may enroll in or change coverage only if they have a qualifying event that creates a Special Enrollment Period.

Key Open Enrollment dates
    • Nov. 1, 2024 – Open Enrollment begins – choose your 2025 plan

    • Dec. 18, 2024Deadline to apply for coverage which begins Jan. 1, 2025

    • Dec. 31, 2024 – Last day of coverage for your 2024 plan

    • Jan. 1, 2025 – 2025 coverage begins; first 2025 monthly payment is due

    • Jan. 15, 2025 – Last day of Open Enrollment

Note: If you enroll Dec. 19, 2024 – Jan. 15, 2025, your coverage will begin Feb. 1, 2025.

2025 benefit highlights*

  • With Quartz’s Virtual First benefit, members will have a $0 copay for telehealth and virtual appointments for primary care, behavioral health, and urgent care (included on all plans except HSA and Standard Plans)
  • Members with chronic conditions can receive self-monitoring tools at no additional cost to help manage their health, with dedicated clinicians offering support. This program includes a blood pressure monitor or scale, along with clinical supervision and regular reminders to stay on track
New ways to save:
  • Our new 6-tier formulary design for non-Standard Plans features $0 cost for preventive medications (tier 1) – Quartz Formulary Information
  • The Gold Maintenance Plan includes access to personal Durable Medical Equipment (DME)* like CPAP, hearing aids, and nebulizers with no out-of-pocket costs. Prior authorization may apply
  • Quartz offers a Health Savings Account (HSA) eligible High Deductible Health Plan (HDHP) Bronze Plan. You can use an HSA pre-tax savings account to help cover out-of-pocket health expenses such as copays, dental exams, or even health care materials like eyewear or hearing aids
Dental and vision highlights:
  • Many Gold, Silver, and Bronze plans are also available, with options to add dental and adult vision benefits
  • Vision benefits include material coverage with a $100/year annual benefit for eyewear such as glasses or contacts

Option 1: Quartz Marketplace plan

By enrolling in a Quartz Marketplace plan, you may be eligible for additional cost savings.
    • You may qualify for enhanced tax credits, which help lower your premium. Be sure to log into your Marketplace account to update your income to receive the most accurate amount of tax credit savings.

    • Cost-share reduction plans lower your out-of-pocket costs, such as deductibles and copays, when you seek covered services.

Currently covered directly by Quartz?
    • If you’re interested in a new plan for 2025, you can explore your Quartz coverage options on the Marketplace.

    • You may now be eligible for tax credits to lower your monthly premium.

Option 2: Quartz direct plan

For individuals and families who don’t quality for tax credits through the Marketplace, Quartz has plans that you can purchase directly.

You can only qualify for cost-saving subsidies if you apply for a Marketplace plan. Quartz direct plans are not eligible for tax credits through the Marketplace.
