Answers to common member questions

  • How do I find my ID card?
    Quartz members receive their member ID card in the mail. You can also print a copy of your ID card in Quartz MyChart by going to “Menu,” “Print an ID card,” and clicking “print card.” If you have not received yours yet, we recommend contacting Quartz Customer Success or submitting a Quartz ID Card Inquiry in MyChart.
  • Where do I find my benefit information, such as premium, deductible, co-pays, and out-of-pocket maximum?
    Your Quartz MyChart account is your “one-stop-shop” for information about your benefits. In MyChart under Menu, click “Coverage,” “View coverage details, deductibles, accumulators,” and then “Benefit Overview” to learn more about your premiums, deductibles, co-pays, and out-of-pocket maximum.
  • Where can I find my pharmacy benefits?
    If your plan includes pharmacy benefits, you can read about them by logging into Quartz MyChart. Under the Menu, simply click on “Pharmacy Benefits – QuartzRx” to access the pharmacy benefits tool. You can also visit our Pharmacy Coverage page to find your plan’s formulary and frequently asked questions about pharmacy benefits.
  • What services are considered preventive care?
    Preventive care services are recommended to check overall health, identify risks, and learn ways to stay healthy. These typically include yearly physical exams, screenings, and immunizations. Check out our Annual Wellness Guidelines page for recommended preventive screenings and the essential information and resources section of our Forms and Resources page for a list of preventive services covered under the Affordable Care Act. You can also contact Quartz Customer Success if you have questions about preventive care.
  • What services are considered maintenance care?
    Maintenance care includes services your provider recommends to help you feel more comfortable or to prevent future issues, but are no longer considered medically necessary. These are often follow-up services, such as chiropractic, cardiac rehab, physical, occupational, and speech therapies. If you’re unsure whether your services are considered maintenance care, contact Quartz Customer Success for assistance.
  • What virtual visit options are available?
    Quartz members have access to virtual visits through virtual care apps and websites for urgent, non-emergency health issues. You’ll connect with a medical professional who may diagnose you, suggest follow-up care, and prescribe medication when appropriate. Visit our Digital Tools page to learn more about virtual visits and create your account so you’re ready when you need care.
  • Can I use my insurance outside of my state or country?
    Quartz members can get urgent and emergency care anytime, anywhere in the United States. For international coverage, review your plan documents and contact Quartz Customer Success for more information.
  • What is the availability and wait times for providers?
    Please contact your provider’s office directly for the most up-to-date information on wait times and availability. To find a provider in our network accepting new patients, use our Find a Doctor tool and filter for providers accepting new patients.
  • Does Quartz require referrals?
    No. Quartz does not require a written referral to see an in-network specialist. However, it is recommended that you contact your primary care provider (PCP) before you seek specialty care. Some health systems may require you to see your PCP first before making specialty appointments.
  • Does Quartz require prior authorizations?
    Yes. Quartz does require prior authorization (PA) for some services or if you receive care out-of-network. Your provider must submit a PA to us for review before services are received. Visit our Prior Authorization Resources for Members page to learn what procedures, services, or medications require prior authorizations on your health plan.
  • How can I update my personal information (address, phone number, etc.)?
    You can update your address, phone number, and preferred first name in Quartz MyChart under your Personal Information. You can also email or call Quartz Customer Success to have them update your address, phone number, and legal name. They will, however, not be able to update a preferred name on your account.
  • How do I change my plan or update my coverage (life event, removing/adding someone to the plan, etc.)?
    If you need to make changes to your plan or update your coverage, contact your Human Resources Department or employer. Your employer will send the updates to Quartz so that your plan or coverage can be updated.
  • How can I get a cost estimate for medical or pharmacy services?
    With the Cost Estimator* tool in Quartz MyChart, you can get an estimate to compare the costs for medical and pharmacy services from the comfort of your home. The tool gives you the power to research and plan before committing to services, saving you time and money while you make important medical decisions. Alternatively, you can contact Quartz Customer Success to have them help with a cost estimate.

    *This estimate is not a guarantee that benefits will be provided for the item or service. It is subject to change based on enrollee-specific characteristics and actual services received, and may not reflect the amount ultimately charged. For the most accurate and up-to-date details, contact Quartz at the number on the back of your member ID card.

  • What is the status of my claim or reimbursement?
    Check your Quartz MyChart account for a list of all your claims and EOB statements that are currently available. If a claim is not yet available in MyChart, contact Quartz Customer Success or send them a message in MyChart to inquire about it.

  • What should I do, or how do I request a refund if I am charged the full amount for virtual visits (e.g., the UW Health Care Anywhere app via UW Health)?
    Contact Quartz Customer Success to have the claim reviewed. If it needs to be corrected, we will ensure it is reprocessed. However, if the provider charged you more than the patient total, you will need to reach out to the provider directly for reimbursement.

  • How does filing an appeal work?
    Quartz welcomes member input. We work hard to resolve problems our members share with us. If you are dissatisfied with your plan, you have the right to share your thoughts. Visit our Appeals and Grievances page to learn more about the appeals process.
