Who is eligible to receive a no-cost blood pressure monitor?
Quartz members diagnosed with hypertension (high blood pressure) or another qualifying condition are eligible to receive an automatic blood pressure monitor at no cost. Medicare Supplement, Medicare Select, and Quartz Align members are not eligible. Contact Quartz Customer Success for questions.
* Quartz Well Rewards is not available to Quartz Medicare Advantage (HMO), Quartz Dual Eligible, Medicare Supplement, Medicare Select, Quartz BadgerCare Plus and/or Medicaid SSI, and State of Wisconsin Group Health Insurance Program members.
