Your Quartz Care Management team makes it easier to find the care and resources you need at no extra cost to you. They can help with:
- Transitions of care and finding local health resources
- Providing personalized guidance to prevent or manage health conditions like diabetes and heart disease
- Supporting your behavioral and mental health needs, including help managing stress, anxiety, and depression
- Finding prenatal and postpartum support and resources
- Answering your physical, behavioral, and mental health questions, guiding you to make informed choices that support your whole-person well-being

What is Care Management and who is it for?
Our Care Managers are nurses and health professionals who can help you navigate health challenges, answer questions about care, and connect you to health resources. We partner with you and your care team to support your physical, mental, behavioral and social well-being. Care Managers are here to help all Quartz Members.
How can Care Management help me?
Care Managers provide an extra layer of support to help you understand and coordinate your health care. They’re here to help by:
- Providing personalized support to manage conditions like diabetes, heart disease, anxiety, depression, and stress
- Teaching you about your health so you can make informed choices
- Connecting you with trusted community resources
- Help you better understand your medications and why you’re taking them
- Working with Quartz services, health providers, and community groups to build a strong support system for you
- Coordinating your care to address medical, social, behavioral and mental health needs with a short-term, focused plan
Are Care Management services covered for all Quartz members?
Care Management services are available to Quartz members with primary coverage through Quartz. These services are usually not available for Quartz Align members or those who are in a skilled nursing facility, hospice, or currently hospitalized.
Why is Care Management calling me?
Our members’ well-being is our priority. Care Management may reach out to select members to offer assistance, check in on transitions of care, and explore support and services available to you.
Does the Care Management team offer support for mental health?
Absolutely. Our Care Management team provides guidance for a wide range of mental health needs, such as anxiety, depression, bipolar disorder, and panic or compulsive disorders. We’ll work to understand your needs, answer questions about available services, assist in coordinating care between multiple health providers and levels of care, and help you create a plan to reach your personal goals. We’ll also help you outline practical steps toward your personal goals. Families of children experiencing mental health challenges can also access this support at no extra cost.
Does the Care Management team offer support for substance use?
Yes. If you or a loved one are dealing with concerns around alcohol or drug use, our Care Management team is here to help. We’ll listen to your situation, guide you toward treatment options and community resources, and coordinate care between providers when needed.
How is Care Management different from Customer Success?
The Care Managers are professionals who help you manage your health and advocate for your needs. We work with you to create personalized care plans, guide you through the health care system, and connect you with the right resources.

Can Care Management help with scheduling or expediting appointments?
Care Managers don’t schedule or expedite appointments, but we’re here to help in other ways. We can connect you with helpful resources and guide you through the health care system by explaining your options so you can make informed decisions about your care.

How long can Care Management support me?
Care Managers support you for as long as you need it. For ongoing health conditions, we provide long-term coordination and resources. For short-term needs like recovering from a procedure or navigating a new diagnosis, we can help create focused plans and connect you with the right resources to address your immediate concerns.
