Quartz is committed to offering a high-quality provider network within our service area. If you would like us to consider adding your provider entity to our network, please complete a Provider Participation Request form and include:
- A link to your website
- Detailed information in the open text fields
Our evaluation process considers multiple factors, including (but not limited to):
- Medicare/Medicaid certification
- Network adequacy
- Provider status (full-time vs. part-time)
- Availability to accept new patients
- Telehealth vs. in-person service offerings
- Regulatory compliance
- Quality of care standards
Please allow up to 30 days to review your request form. You will receive an email notification once the review is complete, indicating whether your request has been approved or denied.
