Quartz Small Group health plans are thoughtfully designed in partnership with local health care providers to offer your employees access to a trusted network of medical professionals, ensuring high-quality care when they need it most. These comprehensive plans go beyond standard coverage by offering a range of beneficial programs to address chronic conditions, wellness, behavioral health, preventive services, and more.
Quartz One Achieve Network - Illinois
Under the Quartz One Achieve Network, you have direct access to over 3,000 physicians and specialists throughout Illinois, Wisconsin, Iowa, and Minnesota featuring local top-quality care your group deserves, including:
- UW Health/SwedishAmerican
- Aurora Health Care
- Freeport Health Network (FHN)
- Beloit Health System
- Katherine Shaw Bethea (KSB)
In addition, most specialist visits will not need a referral – including specialist visits at UW Health/SwedishAmerican.
The Quartz One Achieve Network is available to employer groups in the following counties:

Quartz One Achieve Plus Network - Illinois
Quartz brings northern Illinois increased patient access to UW Health SwedishAmerican Hospital, a division of UW Health. Together, Quartz and SwedishAmerican are taking patient-first care to a whole new level. UW Health North Main Partners Health Center introduces a new, convenient, and personalized care model to members enrolled in a Quartz One Achieve Plus plan.
In addition:
- Employers who choose a Quartz One Achieve Plus plan will stabilize their benefit costs while protecting valuable human resources.
- Employees will enjoy a health care experience like no other when they select Partners Health Center as their primary care provider and medical home.
- All services, including lab work and X-rays, are covered at 100% with no copays or cost-sharing.
And, employers who have Quartz administer their Self-Funded plan can offer the Partners Health Center experience to their employees.

National Network - PPO
Quartz offers PPO plans to provide national network access options for members who live in the United States outside of our Illinois service area. Quartz PPO (Cigna PPO Network*) covers a majority of our groups along with Multiplan’s Health EOS network and PHCS.
