Large Group networks and service areas

These comprehensive HMO, PPO, and POS plans go beyond standard coverage by offering a range of beneficial programs to address chronic conditions, wellness, behavioral health, preventive services, and more.
Quartz One Network - Illinois
Our expansive Quartz One regional network includes providers throughout Illinois and Southwest and South Central Wisconsin. We make it easy for your employees to find the care and coverage they need. Under the Quartz One Network, you have direct access to over 3,000 physicians and specialists throughout Illinois, Wisconsin, Iowa, and Minnesota featuring local top-quality care your group deserves, including:
- UW Health/SwedishAmerican
- Aurora Health Care
- Freeport Health Network (FHN)
- Beloit Health System
- Katherine Shaw Bethea (KSB)
In addition, most specialist visits will not need a referral – including specialist visits at UW Health/SwedishAmerican.
Illinois Performance Plus Network
The Illinois Performance Plus Network includes access to the UW Health North Main Partners Health Center. Employees and their dependents who choose Partners Health Center as their PCP will experience a new model of patient-first care.

National Network - PPO
Quartz offers PPO plans to provide national network access options for members who live in the United States outside of our Illinois service area. Quartz PPO (Cigna PPO Network*) covers a majority of our groups along with Multiplan’s Health EOS network and PHCS.
