The early days

Our story begins with the formation of the health plans in Wisconsin that would later become Quartz

1983

HMO of Wisconsin Insurance Corporation forms as one of the nation’s first rural-based HMOs

1994

HMO of Wisconsin Insurance Corporation and U-Care HMO, Inc. become Unity Health Plans Insurance Corporation (Unity)

1995

Gundersen Health Plan, Inc. (GHP) incorporates in Wisconsin as a non-stock service insurance corporation

Strengthening of partnerships

The growth of these health plans led to full ownership from provider sponsors and ultimately the decision to merge the health plans

2005

Unity becomes a wholly owned subsidiary of UW Health

2016

Unity and GHP combine operations, co-owned by UW Health and Gundersen Health System

2017

Unity, GHP, Physicians Plus Insurance Corporation (PPIC), UW Health, UnityPoint Health – Meriter, and UnityPoint Health (UPH) formalize plans to collaborate, UPH becomes a company owner

Formation of Quartz

The merging of the health plans led to the creation of one brand to encompass these entities

2017

The Quartz brand is introduced, integrating Unity, GHP, and PPIC

Continued growth 

The addition of a new provider sponsor and additional lines of business highlight Quartz’s mission to build proactive solutions together for a life well-lived

2021

Advocate Aurora Health becomes a minority owner in Quartz Health Plan Corporation

2022

Quartz announces Dual Eligible and Medicaid SSI plans to expand coverage in our communities.

2026

Quartz launches the Quartz Level Funded product for small group employers
