Billing with proper taxonomy code

A taxonomy code is a unique, 10-character alphanumeric code used to identify a provider’s specific specialty and type. It is associated with the provider’s National Provider Identifier (NPI) number and ForwardHealth. To see what taxonomy is linked to your NPI, please review the NPPES NPI Registry.

It is important to note that if you are enrolled as a Medicaid provider, the taxonomy on file with ForwardHealth needs to be on the submitted claim to Quartz. If the taxonomy does not match what
Forward Health has on file, it may cause the claim to be rejected and/or denied. To verify the provider’s taxonomy code with Forward Health, log in to the ForwardHealth Portal.
