Quartz Dual Eligible Rx Drug Coverage

Extra Help for Part D drugs

If you qualify for Low-Income Subsidy (LIS), also known as “Extra Help,” you will pay reduced cost-sharing for your Part D drugs.

Depending on your level of LIS, your share of the cost will be one of the amounts shown below:

  • Generics: $0/$1.45/$4.15/15%
  • Brand/other: $0/$4.30/$10.35/15%

Transition fill

This program allows you to keep taking your medicine if it’s not on your new plan’s drug list or if it’s restricted in some way. Go to QuartzBenefits.com/MATransitionFill for more details.

Insulin savings

You won’t pay more than $35 for a one-month supply of each insulin product covered by our plan, no matter what cost-sharing tier it’s on.

Preventive vaccines

Our plan covers most Part D vaccines at no cost to you, even if your plan has a deductible and you haven’t paid it. Go to our Drug Coverage page on our website or call a Quartz Champion for more information.

To see if a drug is covered, you can search our formulary (a list of drugs our plans cover) at QuartzBenefits.com/MAformulary.
