Wisconsin: Brown, Calumet, Florence, Kenosha, Kewaunee, Manitowoc, Marinette, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, and Winnebago counties.
Gundersen Quartz Medicare Advantage:
Iowa: Allamakee, Clayton, Fayette, Howard, and Winneshiek counties.
Wisconsin: Buffalo, Chippewa, Crawford, Eau Claire, Jackson, Juneau, La Crosse, Monroe, Pepin, Trempealeau, and Vernon counties.
Gundersen MN Quartz Medicare Advantage:
Minnesota: Fillmore, Houston, Wabasha, and Winona counties.
UW Health Quartz Medicare Advantage:
