Enroll in Quartz programs for well-living at no extra cost as part of your plan’s benefits, including Quartz’s obesity and Type 2 diabetes reversal program powered by Virta, tools for mental well-being and 1:1 support with AbleTo, and more.

Find a doctor

Quartz gives you access to trusted providers in your community. There is $0 out of pocket for preventive care*, and you can see a specialist when needed, often without a referral.

In-network local hospital and outpatient sites include:

  • American Family Children’s Hospital
  • Aurora Health Care
  • Gundersen Health System
  • UW Health Hospital
  • UnityPoint Health – Meriter

*Under the Affordable Care Act, some preventive health care services, including many medications, are covered at no cost to you when delivered by a provider within your plan’s network. However, if a new or existing health problem is discussed during a preventive health care visit, a separate office visit will be billed as required by national coding and billing guidelines. You may be subject to out-of-pocket charges (copay, coinsurance, or deductible) for these additional services.

Manage your medications​

Our pharmacy program gives you access to our online tool. Shop for the best prices, find convenient pharmacies, review your medication history and more. You can also set up mail order delivery with Optum RX for a convenient way to manage your medications.

Get one-stop access to all your benefits information with Quartz MyChart. Log in to view your claims, see care information, and send a message to Quartz Customer Success. Available on your computer or mobile device for easy access anywhere.

Lab & X-ray copays​

Getting diagnostic care shouldn’t be a barrier to getting the care you need for a healthier lifestyle. That’s why most of our plans have set copays for laboratory and X-ray services.

Virtual First puts member first​​

Virtual First* is a convenient way to get care while saving time and health care costs.

Members will have a $0 copay for telehealth and virtual appointments for primary care, behavioral health, and urgent care.

Vision benefits​

Coverage focused on keeping your eyes healthy. The adult vision benefit* includes an annual eye exam and $100 in materials for purchasing glasses or contacts.
