Quartz gives you access to respected providers in your community. There is $0 out of pocket for preventive care*, and you can see a specialist when needed, some without a referral.

*Under the Affordable Care Act, some preventive health care services are covered at no cost to you when delivered by a provider within your plan’s network. However, if a new or existing health problem is discussed during a preventive health care visit, a separate office visit will be billed as required by national coding and billing guidelines. This means you may be subject to out-of-pocket charges (copay, coinsurance, or deductible) for these additional services.

Rx Value Tier
Quartz’s Rx Value Tier* benefit provides a lower cost share on select medications that have been shown to help improve overall health when taken daily for certain chronic conditions. This means the cost share of the medication is based on the value of keeping our members their healthiest while managing a chronic condition.
