Having a series of shots can help keep your children from getting many harmful diseases. These shots, called immunizations, help your children build immunity.

Doctors have developed a schedule that shows when children need to get their immunizations.

Before children can go to daycare or school, their immunizations must be up-to-date. Be sure to check with your state's Department of Health Services for the school and daycare immunization requirements in your state.

Fill it In and Win!

Fill it in and Win rewards members for being up-to-date on their immunizations.

Children have two chances to win a cash prize.

  • The first is a drawing for fully immunized two-year-old members. The drawing includes a chance to win cash prizes. Quartz sends members a Fill it in and Win! Entry Card when they turn 19 months old.
  • The second chance to win is at age 11. Preteens are sent a Fill it in and Win Entry Card at 11 years of age. Just fill in the information and make a copy of the card for your records.

If your child just turned 19 months old or 11 years old and has not yet received an entry card, you can print a copy. Otherwise, contact us at (866) 884-4601 to request an entry card.


Quartz has two prize drawings each year per age group. Only fully completed Entry Cards are entered in each drawing.

  • Summer drawing: all Entry Cards received by June 15.
  • Winter drawing: all Entry Cards received by December 15.

Now and then, we receive questions on the safety of immunizations (also known as vaccines) for children. This question has been studied very thoroughly. The Centers for Disease Control and Prevention (CDC) has a webpage that provides safety information about vaccines / immunizations.

  • Iowa Immunization Registry (IRIS) – IRIS keeps track of immunizations online. This is very helpful if you need this information for schools or sports. You can also get this information by calling your clinic.
  • Minnesota Immunization Information Connection (MIIC) – MIIC keeps track of immunizations and requests are available online. This is very helpful if you need information for schools or sports. You can also get this information by calling your clinic.

Subscribers age 18 and older can earn $100 for single plans. Family plans offer $100 for the subscriber and $100 for the subscriber's spouse (or domestic partner).
Due to overlap with the Well Wisconsin Program, the Quartz Well reward program is not available to State of Wisconsin Group Health Insurance Program members.
The Quartz Well reward program is not available to all members. Please check your member materials, log in to MyChart to view your benefits or call Customer Service to verify eligibility. Federal Employee Health Benefits (FEHB) members are not eligible for Quartz Well.
