The Diabetes Health Management Program offers support that can help you enjoy life by managing your diabetes symptoms.

The Diabetes Health Management Program helps eligible Quartz members self-manage their diabetes. The program offers support, learning opportunities, resources and reminders that help you enjoy life by managing your diabetes symptoms.

About Diabetes

Diabetes refers to a group of diseases that affect how your body uses blood sugar (glucose).

Insulin is a hormone that helps the body's cells use sugar for energy. It also helps the body store extra sugar in muscle, fat, and liver cells. Without insulin, this sugar can't get into your cells to do its work. It stays in your blood instead. Your blood sugar level then gets too high.

Most Common Types of Diabetes

Type 2 diabetes

  • A chronic condition that affects the way the body processes blood sugar.

Type 1 diabetes

  • A chronic condition in which the pancreas produces little or no insulin.

Prediabetes 

  • A condition in which blood sugar is high, but not high enough to be type 2 diabetes.

Gestational diabetes

  • A form of high blood sugar affecting pregnant women.

To enroll you in the Diabetes Health Management Program, we may use information from –

  • You (sign up above), your doctor or nurse
  • Your health insurance claims.

Not all Quartz members are eligible for Health Management Programs. Please check your plan benefits.
Self-Management

It's important to take care of yourself – especially if you're dealing with diabetes.  Here are a couple of resources to help.


The Quartz Well reward program is not available to all members. Please check your Member Packet or call customer service to verify eligibility. Badgercare Plus members are not eligible for QuartzWell.
Due to overlap with the Well Wisconsin Program, the Quartz Well reward program is not available to State of Wisconsin Group Health Insurance Program members.
Subscribers age 18 and older can earn $100 for single plans. Family plans offer $100 for the subscriber and $100 for the subscriber's spouse (or domestic partner).
