Quartz Medicare Select Plans​

What is Medicare Select?

Medicare Select is an HMO plan that helps fill in the gaps of original Medicare.

Medicare Select is designed to work with Medicare and may pay some or all of approved charges. This is an individual plan. If you and your spouse both want supplemental coverage, you need to purchase two policies. In order to apply for Medicare Select, you need to be enrolled in Medicare Part A and B.

About Medicare Select
  • Covers Part A and Part B deductibles and helps fill in the gaps between Medicare and your health care expenses
  • Covers a routine eye exam and a routine hearing exam once per year
  • Provides access to a comprehensive provider network with guaranteed access to the world-class physicians at UW Health
  • Coordinates your care through a primary care provider selected by you
  • Does not require written referrals to see specialists; however, some prior authorizations are required, depending on the service
  • Offers value-added programs and services to help you maintain a healthy lifestyle

You are eligible to enroll in Quartz Medicare Select if:

  • You are disabled or age 65 or older
  • You are enrolled in Medicare Parts A and B
  • You are a Wisconsin resident on the initial effective date of the policy

You cannot be covered by Medicaid (BadgerCare) or a Medicare Advantage plan and also have Medicare Select.

Open Enrollment Periods

Each Medicare beneficiary has an Open Enrollment Period. During this time, enrollment in a supplement plan from Medicare is guaranteed. A supplement plan cannot turn down the applicant’s enrollment request (as long as all eligibility criteria are met).

This period lasts for the six calendar months immediately following enrollment in Medicare Parts A and B.

Guarantee Issue Periods

Guarantee Issue occurs if you are applying within six months of enrolling in Medicare Part B or within six months of turning 65. Other Guarantee Issue Periods may apply.

Other Enrollment Periods

If an individual is enrolling outside of either an Open Enrollment or Guarantee Issue period, the application will be reviewed to determine whether the plan will accept the applicant based on health factors.

Quartz Medicare Select effective dates

Does Quartz Medicare Select cover Medicare Part D?

Quartz Medicare Select does not include Medicare Part D (prescription drug) coverage. If you are interested in a separate Medicare Part D plan, we can provide information about companies that provide standalone plans. Some Quartz Medicare Advantage (HMO) plans include Part D coverage. Contact your agent for more information about Part D coverage.

You can find a list of participating providers (doctors, hospitals, and other health care facilities) by going to QuartzBenefits/FindaDoctor (select the Commercial HMO/POS network), or in the Medicare Select Provider Directory.

 

A Primary Care Provider (PCP) is a doctor (MD), physician’s assistant (PA), or nurse practitioner (NP) who manages your health care so you receive consistent medical care in an effective and efficient manner. Your PCP will coordinate your medical care through the use of participating specialty practitioners.

Quartz requires all members to have a PCP.

To select your PCP, Quartz provides you with the following options:

  • Choose a PCP from the list of participating practitioners found at QuartzBenefits.com/FindaDoctor (select the Commercial HMO/POS Network), or in the Medicare Select Provider Directory.
  • Choose a participating clinic, and Quartz will assign a PCP at the clinic.
  • Have Quartz assign you a PCP and clinic close to your home.
  • If you would like help selecting a UW Health PCP, contact the UW Health Welcome Center at (800) 552-4255, Monday through Friday, from 8 a.m. to 5 p.m. You can also send an email to [email protected].

Yes. You can change your PCP at any time. We offer two convenient ways to change your PCP:

  • Go to QuartzMyChart.com
  • Call Quartz Customer Success at (800) 362-3310.

PCP changes are effective on the day of Quartz’s receipt of your request, unless you request a future date.

You should talk with your PCP before seeing other types of practitioners, so he/she may coordinate your health care.

You should work with your PCP to determine if you need specialty care. If you do, he/she will refer you to an appropriate specialist who is participating in your network.
