Open Enrollment

Open Enrollment is an annual time period when individuals can review their plan options and make any needed changes for the following year.
Outside of Open Enrollment, individuals may enroll in or change coverage only if they have a qualifying event that creates a Special Enrollment Period.

Key Open Enrollment dates
    • Nov. 1, 2025 – Open Enrollment begins – choose your 2026 plan

    • Dec. 15, 2025 – Deadline to apply for coverage which begins Jan. 1, 2026

    • Dec. 31, 2025 – Last day of coverage for your 2025 plan

    • Jan. 1, 2026 – 2026 coverage begins; first 2026 monthly payment is due

    • Jan. 15, 2026 – Last day of Open Enrollment

Note: If you enroll Dec. 19, 2025 – Jan. 15, 2026, your coverage will begin Feb. 1, 2026.

2026 Benefit & coverage highlights

Easy access to care
  • With Quartz’s $0 “Virtual First” benefit (where available), non-HSA members non-HSA members will have a $0 copay for telehealth and virtual appointments for primary care, behavioral health, and urgent care, which includes consultation, diagnosis, follow-up care, and prescription services
  • Members with chronic conditions can receive self-monitoring tools at no additional cost to help manage their health with dedicated clinicians offering support. This program includes a blood pressure monitor or scale, along with clinical supervision and regular reminders to stay on track
Ways to save
  • $0 cost preventive maintenance medications with an easy formulary design and convenient online formulary tool
  • Access to personal Durable Medical Equipment (DME) like CPAP, supplies, hearing aids, and nebulizers with no out-of-pocket costs when on the Quartz Gold Maintenance plan. DME is also available on other plans with fixed coinsurance options. (Prior authorization may apply. Diabetic test strips, glucometers, and insulin syringes are covered under the pharmacy benefits and may apply to applicable copay, deductible, and coinsurance.) 
  • Health Savings Accounts (HSA) may be paired with Bronze and Catastrophic plans. Members can add an HSA to their 2026 on exchange or off exchange Bronze plan even if the plan has a $0 deductible or copays for services with the new federal tax guidance
Dental and vision
  • Many dental and vision options are offered across select Gold, Silver, and Bronze plans
  • Vision benefits include material coverage with a $100/year annual benefit for eyewear such as glasses or contacts

Explore, enroll, or renew a health plan – you’ve got options

If you choose to renew your current Quartz plan, no action is needed. You’ll be automatically renewed based on the information provided in your renewal packet. If you want to explore or consider other Quartz options, please consider the two options below.

Important: Due to federal changes in tax credit calculations and availability, you may notice higher premiums than expected in 2026. If you think you may be eligible for tax credits, you must enroll through the Health Insurance Marketplace or through MNsure and also update your information. Required details, like income or household changes, are needed to help determine tax credit eligibility.
