My Quartz Tools

Our new provider portal, My Quartz Tools, was introduced in early March. All former MyPlanTools users should have received a username and a temporary password for My Quartz Tools. If you have not done so already, please log in and establish a new password. If you did not receive this information or are having problems getting started, call Customer Service at (800) 362-3310.

Provider Spring Regional Meetings

Quartz will be replacing our usual face-to-face meetings with a series of webinars. Stay tuned for invitations and further details.

Form and Resources

Looking to update or terminate a provider/location? Check out our provider forms to find the resources you need. There are also additional resources available such as prior Authorization forms, EDI, the provider manual and much more.

Code Edits for ASC Claims

To gain efficiency and accuracy in editing Ambulatory Surgical Center (ASC) claims, we are implementing a new set of code edits designed for the specific billing and coding guidelines of ASCs. This change is scheduled to take effect by March 21, 2020, and will impact UB04 or CMS1500 claim forms.

For your reference, see the CMS MLN Matters Ambulatory Surgical Center Payment System Fact Sheet ICD 006819 Jan 2019.

Medicare Part D Vaccination Reminder

Under Medicare Part D, many vaccines are not covered if administered in the clinic. Be sure to check coverage before vaccinating so your patient doesn’t incur a cost, which could be avoided by getting the dose at an in-network pharmacy.
