Name Change

We’re changing our Senior Preferred product name to align with the Quartz brand and make being a Quartz Senior Preferred Medicare Advantage (HMO) member even easier. These frequently asked questions and answers will help you understand what this change means to you and your Medicare Advantage plan.


  • Am I still covered?
    Yes. Your plan will continue to be insured by either Quartz Health Plan Corporation or Quartz Health Plan MN Corporation. Your coverage will not change.
  • Is the name Senior Preferred going away?
    Yes. During the next few months, you’ll start seeing the name Quartz Medicare Advantage (HMO) on our website, advertisements, and plan materials.
  • What should I expect?
    • The Senior Preferred logo will be replaced with the Quartz logo.
    • You’ll continue to receive the same great service from the same great people you’ve worked with over the years.
    • Quartz will continue to offer Medicare health insurance as it has for 20 years.
  • What does this name change mean to me as a Medicare Advantage policyholder?
    • As Quartz grows and expands, so do the provider networks and benefits we offer.
    • We also added new preventive benefits and access to our wellness program, Quartz Well.
  • What should I know about Quartz?
    • Quartz was formed when three plans merged: Gundersen Health Plan, Unity Health Insurance, and Physicians Plus. The new company was named Quartz.
    • Quartz is provider-owned by UW Health, Gundersen Health System and UnityPoint Health.
    • As a community-based health insurance company, Quartz serves 342,000+ members in Wisconsin, Minnesota, Iowa and Illinois.
  • Do I have to change doctors?
    No. Not unless you want to. As of January 1, 2020, you have access to all Quartz Senior Preferred network providers. These include the Gundersen Health System, UW Health, ProHealth, and SwedishAmerican. See your 2020 Annual Notice of Change (ANOC) for more details.
  • If I have MyChart, will I see any changes to my account?
    No. Your MyChart didn’t change a bit. It’s still as convenient and secure as ever. Don’t have a MyChart account yet? Signing up is easy at
  • What about auto-pay? Do I need to make any changes?
    No, your payment will continue as normal.
  • What if I receive a document with the old name?
    No big deal. It might take us a while to update all materials, so we appreciate your patience while we work to complete the name change. However, you may continue to see the Senior Preferred logo on some of your documents.
  • Do I still call the same Customer Service number?
    You bet! The Quartz Senior Preferred Medicare Advantage Customer Service number will stay the same – (800) 394-5566 or TTY 711.
  • Will my health plan be serviced by someone else?
    No. Quartz is still Quartz. We’re changing the Senior Preferred product name, not the companies servicing your health plan. We’re still the same organization you know and trust, providing the same exceptional service you deserve.
