With complex case management (CCM), we will act as your advocate. At the same time, we’ll help you become empowered to voice your health care needs and advocate for yourself.

Navigating the health care system can be confusing, especially if you have a serious illness, injury, or many health-related needs. Wouldn’t it be nice to have someone in your corner who can help? That’s where Quartz’s complex case management (CCM) comes in. Here’s how it works.

If you have complex health issues that need extra support, we’ll match you up with a case manager to make sure you get the best care possible. This person, typically a nurse or social worker, will work with you one-on-one over the phone to help you:

  • Learn more about your condition and how to manage it successfully
  • Overcome barriers to care
  • Monitor your ongoing behavioral and physical conditions, and coordinate services and specialties as needed
  • Make the most of our health care coverage
  • Develop an individual case management plan and goals
  • Access community resources for you and your family
  • Enhance your quality of life
  • Gain independence and personal growth
