Go ahead - choose your type of care.

  • Primary Care is regular, non-emergency care. Seek primary care for regular check-ups or problems like the flu, aches, and pains.
  • Urgent Care is for a health issue or minor injuries that need prompt attention but are not serious enough for emergency care. Call your doctor first to check the best course of action.
  • Telehealth/Virtual Visits are two different services with distinct differences.
  • Emergency Care is for serious medical problems such as heart attacks, strokes, and major injuries. Go to the nearest hospital or call 911 in an emergency situation for immediate help.
  • Away From Home Care is urgent or emergent medical care while you are outside of your Quartz network.
  • Specialty Care is health care from providers who focus on certain types of illness or treatments
  • Hospital Care is health services provided in a hospital setting. This type of service may need Prior Authorization.
  • Pharmacy care gives you access to the prescriptions you need.

Start Here – Choose a Primary Care Provider*

If you are new to Quartz, your first step is to choose your doctor, known as your primary care provider (PCP). PCPs manage your care and lead your health care team to help make sure you are getting the right care at the right time.

  • To choose a PCP, visit Find a Doctor. Use your Quartz ID card to find your network; once you select the correct network, you will be able to select your doctor based on name, location or specialty.
  • If you do not select a PCP, Quartz will assign one to you.
  • If you want to change your PCP listed on your ID card, complete the PCP Change Form through MyChart.

Know Your Rights & Responsibilities

As a member, you have the right to choose a doctor* from our PCP network. It is important that you see your PCP regularly. This will help your doctor know the best way to serve your health care needs. See Member Rights & Responsibilities.

If Quartz administers your health plan for your employer, please login to MyChart to view your group's Member Rights & Responsibilities statement within your Summary Plan Description.
Check Plan Coverage and Benefits

No matter what kind of health care you need, it is important to check what your plan covers. Your benefits differ depending on the type of plan you have. A referral or prior authorization may be needed before some services will be covered by your plan. Your Quartz ID card will state what kind of plan you have. Be sure to check coverage before getting medical care other than from your PCP. You can see all your benefits by logging in to MyChart.

Estimating Your Out-of-Pocket Costs

Determine costs you may incur prior to receiving health care services using the Determination of Benefits Form through MyChart. To complete the form, you will have to ask your provider for certain medical coding information including procedure codes, procedure modifiers and unit codes for the services.
