Non-Emergency Medical Tranpsoration

BadgerCare Plus members can use the non-emergency medical transportation vendor, MTM Inc., for non-emergency transportation to medical appointments.

To schedule transportation

  1. Schedule routine rides at least two business days before your visit
  2. Call 866.907.1493 to schedule a ride, or
  3. Go online and schedule a ride

If you call less than two business days before your appointment, you might not get a ride – unless it’s urgent. If you can not get a ride, you will have to reschedule your appointment and the ride.

Otherwise, you can ask the clinic to schedule the ride for you when you make your appointment. A ride to an urgent appointment will be provided in three hours or less.

To file a complaint –

Call MTM, Inc.’s “WeCare” line at 866.436.0457 if you have a complaint about your service. You may also use their online form. Providing excellent service is important to them. They will follow up on all complaints. They will contact everyone who was part of your scheduling and transport.

If your ride is late –

Contact MTM, Inc.’s “Where’s My Ride” line at (866) 907-1494 if –

  • You have been waiting for your ride for more than 15 minutes after your scheduled pick-up time
  • You had a pre-scheduled return ride and you have waited for more than 15 minutes
  • You called the transportation provider after your appointment and have waited for more than one hour

If you have questions about your ID card or you need a new ID card, call ForwardHealth Member Services at (800) 362-3002.

Enrollment

If you have enrollment questions, call a ForwardHealth HMO Enrollment Specialist at (800) 291-2002.

Pharmacy

Some drug stores accept ForwardHealth ID cards. If so, members can get covered prescriptions. They may also cover some other medicines.

Dental, Chiropractic, and Autism Treatment Services

If you want to talk to someone outside Quartz about the problem, call the HMO Enrollment Specialist at (800) 291-2002. The HMO Enrollment Specialist may be able to help you solve the problem. HMO Enrollment Specialist can also help write a formal grievance to Quartz or to the BadgerCare Plus program.

If your complaint or grievance needs action right away because a delay in treatment would greatly increase a risk to your health, please call Quartz as soon as possible at (800) 362-3310.

You will not be treated differently from other members because you file a complaint or grievance. Your health care benefits will not be affected.

Appeals

You have the right to appeal to the State of Wisconsin, Division of Hearings and Appeals (DHA), for a fair hearing if you believe your benefits are wrongly denied, limited, reduced, delayed, or stopped by Quartz.An appeal must be made no more than 45 days after the date of the decision being appealed. If you make an appeal before the effective date, the service may continue. You may need to pay for the cost of services if the hearing decision is not in your favor.

The hearing will be held with an administrative law judge in the county where you live. You have the right to be represented at the hearing or you can bring a friend for support.
