Call the federal Health Insurance Marketplace at (800) 318-2596, or you can call a health insurance company directly. Quartz’s friendly and knowledgeable customer service staff are standing by at (608) 644-3430 (Monday – Thursday from 7 a.m. to 6 p.m. and Friday from 7 a.m. to 5 p.m.).

By Mail

You may go to the Health Insurance Marketplace and download a paper application. Follow the directions included in the application to fill it out and send in the completed form.

In Person

Visit to find a list of places where you may meet with someone directly. You may also find a health insurance agent by going to the Wisconsin Office of the Commissioner of Insurance and searching for a licensed agent or agency. Or, you may call Quartz at (800) 362-3310 and set up an appointment with a friendly and knowledgeable representative.
