About the Program

The Quartz Pharmacy Program is based on local medical practice patterns and managed following NCQA best practices, this program helps provide effective medication management for patients.

Patient Medication Costs

You can determine the copay level (tier) for your patient’s medications by reviewing the Drug Formulary. The formulary lists medications by drug class according to the copay tier.

Certain medications require prior authorization before coverage is provided.

Oral Oncology Split Fill Program

Certain oral oncology medications have the first six fills limited to a 14 or 15 day supply per fill. After six fills, the prescription may be filled for up to a 30 day supply per fill. Copays will be prorated so that participants will only pay half of a copay for the two-week fills.

The purpose of this program is to provide additional monitoring and minimize waste for certain oral oncology drugs. The medications included in this program have a high rate of discontinuation due to adverse effects. Many patients who begin taking one of these medications stop or adjust the medication within the first few weeks of therapy.

Prescriptions for these medications must be filled at a participating Quartz Specialty Network pharmacy and a pharmacist trained in cancer therapies will assist a member through their treatment course.

Medications included in the Oral Oncology Split Fill Program are designated with "ONC" on the formulary listing.
