Enrollment Paperwork

As a reminder, only enrollment changes should be submitted to Quartz for processing. We don’t need paperwork for someone who is not making changes.

COBRA Direct Pay Discontinued

Effective July 1, all COBRA members newly enrolling are placed on their group’s bill. Current COBRA Direct Pay members can continue their status until their coverage is termed.

Updated Applications

Are you using the most current Quartz applications and forms? Please refer to Agents Forms & Resources. Specifically, our Employer Group Application was recently updated.

MyPlanTools Enhancement

Coming soon! In response to agent feedback, we are expanding MyPlanTools to include access to your clients’ Individual and Medicare member information. You should start seeing these plans types in early September.
