As of January 1, 2019, Quartz will transition members under the Gundersen Health Plan, Inc. and Gundersen Health Plan Minnesota licenses to the Quartz Pharmacy Benefit.

Quartz manages the pharmacy benefit to maximize value for our customers. We have a team of pharmacists that reviews the clinical data and collaborates with providers to keep our coverage policies and formulary current, clinically appropriate and value oriented. As part of this change members will be transitioning to the Quartz formulary. Additionally, pharmacy claims will be processed by the pharmacy claims vendor contracted through Quartz.

How Do I Find the Quartz Formulary?

On January 2nd, the links to the Gundersen Health Plan formulary will be replaced with links to the Quartz formulary.  So just navigate to the formulary as you usually do, and you will find the correct formulary.

New ID Cards

In December, members with policies underwritten by Gundersen Health Plan, Inc. and Gundersen Health Plan Minnesota received a new ID card with the new PBM information and a letter that explained that a new PBM would be take effect January 1, 2019.

Members should be sure to present this ID card to their pharmacy for any prescriptions needed starting January 1, 2019 so that the pharmacy can submit the claim to the new pharmacy claims vendor.
