Quartz has a revised Employee Application for new hires. To simplify the enrollment process for applicants who experience a loss of other coverage, an applicant will certify their recent loss of other coverage by “checking the box.” Checking the box under Reason for Enrollment, Loss of Other Coverage, confirms a loss of other coverage that entitles the applicant to a Special Enrollment Period.
