Now is the perfect time to focus on heart health with your patients, especially when it comes to controlling hypertension. As one of the most common chronic conditions in the U.S., hypertension affects over 120 million U.S. adults and 1.4 million adults in Wisconsin. It is the leading risk factor for heart disease, stroke, and chronic kidney disease
Making hypertension a priority
At Quartz, we are committed to making hypertension management a priority. That’s why we’ve partnered with the American Medical Association (AMA) to offer their AMA MAP™ Hypertension solutions and resources to any interested provider at no cost.
How the AMA MAP™ Hypertension program transforms care
This evidence-based quality improvement program is designed by cardiovascular disease experts with the whole care team in mind. It aims to:
- Rapidly improve hypertension control rates
- Sustain progress over time
- Enhance patient outcomes
The AMA is dedicated to improving blood pressure control rates nationwide by working closely with health care organizations, physicians, and clinicians. Their focus is on accurately measuring blood pressure, acting rapidly to intensify treatment, and partnering with patients for better results.
Upcoming resources and support
Throughout the year, we will be sharing educational content, quality improvement resources, and clinical practice tools from the AMA to support you in managing your patients’ hypertension. Watch for future communications filled with valuable resources.
Quartz tools and support for better heart health
In addition to the AMA MAP Hypertension program, Quartz offers additional resources to help you and your patients focus on heart health:
- Home blood pressure monitors: Providers can order a blood pressure monitor for eligible patients by completing our self-monitoring equipment order form. We also encourage you to share QuartzBenefits.com/BPMonitoring with your patients. It provides helpful information on blood pressure levels, good techniques for accurate readings, when to contact the doctor, and more.
- Remote Patient Monitoring (RPM) reimbursement: Quartz will reimburse providers who monitor blood pressure to support hypertension care. For more information, call the Quartz Clinical Programs team at (877) 204-9577.
By making hypertension management a priority, we can work together to improve heart health and enhance patient outcomes. Let’s take the next step toward a healthier future!
