For compliance reasons, Unity Health Insurance can no longer provide member information to health clubs or fitness facilities. Group plan members, who are insured through their employer, will need to submit proof of activity for reimbursement. To receive the reward, Unity-approved health club(s) will need to supply participants with a printout of 60 workouts in six months. The printout and completed Fitness First & More Reimbursement form (available at UnityMyChart.com) should be sent using one of these methods −

To be eligible for reward reimbursement, it is important to −

  • Be a Unity member, age 18 or older.
  • Swipe in or log in at your health club to track your workouts.
  • Exercise at a participating health club 60 times in a consecutive six-month period.
  • Work out at least 30 minutes—only one workout per day qualifies.

Health clubs will no longer submit your workouts to Unity. Submission for reimbursement should be made after achieving 60 workouts in six months. Learn more by logging into your MyChart account at unitymychart.com and accessing the Wellness Portal.
