Stay healthy with preventive care with $0 out-of-pocket costs!*
There is $0 out-of-pocket cost for preventive care. And when you need to, you can see a specialist, some without a referral.
*Under the Affordable Care Act, some preventive health care services are covered at no cost to you when delivered by a provider within your plan’s network. However, if a new or existing health problem is discussed during a preventive health care visit, a separate office visit will be billed as required by national coding and billing guidelines. This means that you may be subject to out-of-pocket charges (copay, coinsurance, or deductible) for these additional services.

What’s an Aggregate Deductible?
Quartz offers HSA plans that have an aggregate deductible. With an aggregate deductible, if the plan covers more than one person, the “per person” deductible does not apply. Instead, the “family” deductible must be met before Quartz will pay benefits. One person’s claims may satisfy the entire family deductible.
The out-of-pocket limit is $8,700 for a single person and $17,400 for a family plan. One person will never accumulate more than $8,700 in out-of-pocket costs per benefit year and the family will never go over $17,400 per benefit year.
