Are You at Risk?

Your genetics make you more prone for getting both Type 1 and Type 2 diabetes. However, Type 2 diabetes is also caused by lifestyle factors.

Resources to Help

The Wisconsin Institute for Healthy Aging offers workshops and classes to support our members living with diabetes, including –
 

  • Living Well with Chronic Conditions is a six-week class for managing one or more chronic conditions.
     
  • Healthy Living with Diabetes is a six-week class that covers topics such as healthy eating, exercise for
    fitness and fun, relaxation techniques and more.

How to Enroll in Quartz’s Diabetes Health Management Program

If you are a Quartz member who has been diagnosed with diabetes, you are automatically enrolled. You may also call the Diabetes Health Management Program Coordinator to enroll. You can reach us toll-free

Program participants receive –

  • A guideline on what types of screenings are recommended
  • Reminders about screening to prevent complications of diabetes
  • A diabetes magazine
